In response to growing global instability, Taiwan faces increasing risks from geopolitical tensions and natural disasters. Anesthesiologists play a pivotal role in trauma and disaster medicine, especially in austere and high-threat environments. To enhance preparedness, anesthesiologists must be familiar with key aspects of emergency care: tiered medical support and initial management, analgesia and sedation under resource-limited conditions, selection of anesthesia techniques, principles of damage control resuscitation (DCR), and effective surgical team coordination. This presentation integrates guidelines from the Joint Trauma System (JTS) and Tactical Combat Casualty Care (TCCC), along with insights from expert experiences during the ongoing war in Ukraine. It aims to provide anesthesiologists with strategic frameworks and clinical approaches to improve resilience and adaptability in crisis settings—ultimately strengthening anesthetic care capacity and optimizing trauma outcomes under fire.
